The opportunity:

We are looking for an experienced Service Coordination Team Leader to lead a team of Service Coordinators delivering in-home care services. In this role, you will support clients to maintain their independence, health, and wellbeing within their community.

Responsibilities include but are not limited to;

  • Develop, manage, and maintain staff rosters to ensure all client services are scheduled appropriately.
  • Allocate Service Coordinators to new care staff to clients based on skills, availability, location, and client preferences.
  • Foster clear and effective communication to strengthen team cohesion and maintain a positive and safe workplace culture.
  • Provide guidance and oversight for the team to ensure delivery of services within the consumer directed care and compliance with the Aged Care Quality standards, guidelines and organisational strategic and business plans.
  • Review and complete incident reports on CRM & VHIMS and oversee complaints/incidents (VHIMS & TP)

Skills & Experience:

  • Minimum of 3 years' experience working In-Home Aged Care Services.
  • Experience leading and supervising teams, particularly in the In-Home Aged Care Services
  • Leadership capability, including sound judgement and a solution-focused approach to problem-solving, with proven experience in guiding and managing teams effectively.
  • Experience working independently within a multidisciplinary team
  • Proven high level of administration and computer skills (specifically MS Office applications, & CRM Systems)
  • Demonstrated ability to engage and communicate effectively communication skills (verbal and written), with diverse audiences to develop and maintain professional relationships across client groups, service providers and internal / external stakeholders
  • A relevant tertiary qualification -Social Work, Allied Health, Nursing, or Community Service
