$110,000 - $120,000 Annual

Key Responsibilies:

  • Assess client needs and develop, implement, and review individualised support plans in line with NDIS goals and participant outcomes. 
  • Deliver tailored support, education, and capacity-building programs to enhance participants’ independence, wellbeing, and community participation.
  • Conduct comprehensive client assessments and interviews to identify the nature and extent of social, emotional, behavioural, and functional support needs. 
  • Monitor participant progress against NDIS plans, maintain accurate case notes and documentation, and prepare reports to evaluate service outcomes and goal attainment.
  • Provide guidance and support to participants and their families, including individuals living with disabilities, across home, community, and supported living environments.
  • Assist clients in addressing social, emotional, behavioural, and financial challenges while promoting autonomy and skill development.
  • Facilitate referrals and link participants to appropriate external services, including health, mental health, housing, employment, and allied health providers, ensuring access to specialised supports in line with NDIS plans.
  • Assess community needs and available services, advocating for participants and supporting access to local resources. 
  • Liaise with community organisations, government agencies, and service providers to coordinate integrated supports and improve service delivery outcomes.
  • Prepare professional documentation, including progress reports, case notes, incident reports, and funding or compliance submissions, ensuring adherence to NDIS Quality and Safeguards Commission standards.
  • Contribute to safeguarding participant wellbeing by promoting rights-based approaches, person-centred practices, and culturally appropriate service delivery.

Requirements:

  • Australian Citizens and Permanent Residents are encouraged to apply. 
  • AQF Associate Degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2) 
  • Minimum of 3 years of full-time, highly relevant executive or senior management experience
  • Proven experience working in disability services, community services, or welfare support
  • Strong knowledge of NDIS framework, policies, and compliance requirements (if applicable)
  • Excellent communication and interpersonal skills, with the ability to engage diverse clients and stakeholders
  • Demonstrated ability to develop and manage support plans and coordinate multiple services
