If  you are passionate about making a difference in the lives of people with special needs? Optimal Care Services is seeking a dedicated Community support Officer to join our team in Bankstown, New South Wales.

Task & responsibilities

  • Identify community service gaps and implement creative solutions, enhancing client independence through targeted activities and programs. 
  • Prepare precise funding submissions for government agencies. 
  • Maintain meticulous, up- to-date client records for progress tracking and compliance, ensuring comprehensive care delivery and documentation. 
  • Monitor clients' well-being, report significant changes. 
  • Provide direct support, addressing immediate needs and ensuring necessary assistance. 
  • Evaluate community resources in health, welfare, housing, and employment. 
  • Develop programs to enhance client independence and overall well-being. 
  • Conduct comprehensive client assessments, develop tailored support plans, and collaborate with healthcare providers to ensure holistic care delivery.
  • Design creative leisure programs that foster social development, effectively manage challenging behaviors, and promote overall well-being.
  • Implement effective care routines, improving clients' daily living experiences and personal grooming while maintaining their dignity. 
  • Conduct comprehensive client assessments, evaluating abilities, interests, and needs to develop personalized care plans that promote independence. 
  • Implement tailored leisure programs, enhancing clients' social development and well- being while managing challenging behaviors through diversional activities. 
  • Partner with healthcare professionals to ensure holistic client support, including medication administration and vital sign monitoring.

Qualification & experience

  • Minimum 1 year experience in a similar role with sound understanding of NDIS guidelines and practices - This is essential. 
  • Excellent communication and interpersonal skills Ability to problem-solve and overcome complex challenges and barriers 
  • Completed at least a Diploma of Community Services qualification from an approved Education provider in Australia. 
  • Hold a current, unrestricted driver’s licence.. 
  • Ensure you have up-to-date immunisations as required, and complete certifications and checks such as a First Aid Certificate, Working with Children Check, Police Check and/or NDIS Worker Screening Check.
