We’re looking for a compassionate and motivated Social Worker to join the Luz All Abilities team in a full-time position. The Social Worker will deliver person-centred social work services that promote the emotional, social and developmental wellbeing of individuals with diverse needs, across all stages of life.


Benefits

The remuneration will be AU$75,000 - 80,000 per annum plus superannuation.


Task & responsibilities

- Assessing the social, emotional and developmental needs of participants with diverse abilities across childhood into adulthood, in a one‑on‑one program format.

- Conducting case interviews and gathering background information from participants, families and carers to identify strengths, support needs and risks.

- Collaborating with coaches, therapists, parents and community stakeholders to design individualised support plans aimed at maximising participants’ abilities and inclusion.

- Facilitating access to appropriate services and resources (health, educational, community, recreational) and acting as liaison between families/participants and external agencies.

- Providing ongoing counselling, advocacy and support to participants and their families or carers, addressing issues such as social isolation, inclusion, behavioural adjustment, self‑esteem and community participation.

- Monitoring participant progress and modifying support strategies as required to reflect evolving needs, goals and outcomes.

- Maintaining comprehensive case records, reports and documentation of interventions, outcomes and referrals in accordance with organisational policies and regulatory requirements.

- Promoting social justice, rights‑based approaches and inclusion by supporting participants to self‑advocate, build social networks and engage in community opportunities.

- Working within the multidisciplinary team of coaches, therapists and educators to coordinate holistic support, share information and contribute to program development.

- Contributing to organisational development by identifying service gaps, participating in pilot initiatives or improvements, and recommending changes to the program to better meet participant and community needs.

- Delivering group or individual sessions (where appropriate) to support life‑skills, social skills, transition planning (eg from childhood to adulthood) and community integration.

- Ensuring that service delivery aligns with the organisation’s philosophy of shifting focus from disability to ability, fostering empowerment, belonging and participation.

- Undertaking risk assessment and risk‑management planning for participants in consultation with carers, therapists and community services, ensuring safe and inclusive participation environments.

- Providing training or mentoring to less experienced staff or volunteers (if applicable) on best‑practice social work approaches, inclusive practice and participant‑centred planning.

- Participating in organisational review processes, supervision, professional development, and contributing to the continuous improvement of service quality and outcomes.


Qualification & experience

- At least a bachelor’s degree in Social Work, preferably a Master Degree in Social Work.

- At least 12 months of demonstrated experience providing social work or equivalent support services to individuals with diverse or complex needs in a centre-based program setting.

- Proven ability to apply person-centred and strengths-based approaches in casework and to collaborate effectively within a multidisciplinary team.

- Excellent interpersonal and communication skills, with the capacity to engage with individuals, families and external stakeholders in a culturally sensitive and inclusive manner.

- Strong organisational skills with experience in maintaining accurate documentation and case records in line with ethical and professional standards.

- A valid Working with Children Check and NDIS Worker Screening Clearance (or willingness to obtain).
