The Community Worker assists individuals, families, and groups by identifying their needs and providing suitable support services and programs. The role includes meeting with clients to understand their social, emotional, financial, or welfare-related issues and creating personalized support plans to help address these challenges.

Task & responsibilities

  • Conduct interviews and assessments to understand clients’ social, emotional, and financial circumstances and identify appropriate support needs. 
  • Develop and implement tailored education, training, and support programs to assist individuals, families, and community members.
  • Monitor client outcomes and maintain accurate records while providing regular progress updates to management and relevant stakeholders. 
  • Connect clients with appropriate community services, agencies, and resources to ensure access to additional assistance and support. 
  • Work collaboratively with community groups, government bodies, and private organisations to address local issues and improve service delivery. 
  • Identify community needs and review available resources related to housing, health care, welfare services, employment, and training opportunities. 
  • Provide guidance and practical support to young people experiencing social, emotional, or financial challenges. 
  • Deliver care and support to children and individuals with disabilities within residential or institutional care settings. 
  • Assist in supervising and supporting individuals on probation or parole when required. 
  • Prepare reports, funding applications, and resource proposals for government departments and partner organisations.

Qualification & experience

  • Relevant qualification in Social Work, Community Services, or a related discipline. 
  • At least 3 years of experience in community engagement, case management, or similar roles. 
  • Excellent interpersonal, communication, and organizational abilities. 
  • Proven capacity to work both independently and collaboratively with a diverse range of clients and stakeholders. 
  • Sound knowledge of community resources, welfare programs, and government services. 
  • Experience supporting children, youth, or individuals with disabilities. 
  • Background in probation, parole, or correctional support services.
